Requirements
- Master of Social Work (MSW) from an accredited institution
- At least 2 years of experience in healthcare, LTC/SNF, geriatrics, behavioral health, or case management
- Strong understanding of social work theories, interventions, and techniques
- A heart for serving individuals from diverse cultural and socioeconomic backgrounds
Responsibilities
- Conduct meaningful assessments to understand residents� needs, strengths, and goals
- Develop individualized treatment plans that honor each resident�s story
- Provide counseling, emotional support, and guidance through life�s challenges
- Connect residents with vital community resources, including mental health services, benefits, housing assistance, and support programs
- Collaborate with nursing, therapy, and interdisciplinary teams to ensure holistic, person-centered care
- Maintain accurate, confidential records that reflect the highest standards of professionalism
- Participate in care conferences and team meetings to advocate for residents� needs
- Stay informed about community programs to better serve residents and families
