About The Position

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. We are seeking a dedicated Social Worker (LMSW/LLMSW) to join our interdisciplinary care team in providing comprehensive services for older adults. In this role, you will coordinate social service programs, conduct in-home assessments, manage community resources, and collaborate with participants’ families to enhance their quality of life.

Requirements

  • Must possess a Master’s Degree in Social Work (MSW) and be licensed in the State of Michigan.
  • Minimum of one year of experience working with frail elderly or long-term care populations.
  • Detail-oriented with strong problem-solving abilities.
  • Excellent written and verbal communication skills.
  • Ability to maintain discretion and confidentiality.
  • Strong organizational skills and proficiency in MS Office Suite (Word, Outlook, PowerPoint, Excel).
  • Current CPR certification by AHA, or willingness to obtain it through Senior Care Partners P.A.C.E.
  • Must be fully vaccinated or willing to become vaccinated against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
  • Must meet CMS competency standards prior to working independently

Responsibilities

  • Conduct in-home comprehensive assessments and develop care plans.
  • Manage community resources effectively.
  • Collaborate with participants’ families and facilitate meetings.
  • Act as an expert in Advance Care Planning.
  • Provide education to alleviate caregiver burden.
  • Demonstrate compassion, integrity, teamwork, and excellence.
  • Be a champion of the Senior Care Partners P.A.C.E. philosophy.
  • Provide exceptional support to all staff, external partners, and participants.
  • Ability to make independent decisions as circumstances warrant.
