The primary function of this role is to provide coordinated care in the home or facility to patients of all age groups. This involves performing psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient and their family. The Social Worker will assess hospice patients to identify psychosocial, grief, financial, environmental, and community needs. They will use effective interpersonal relations and communication skills to facilitate desirable outcomes and meet mandatory continuing education requirements. The role includes attending Inter-Disciplinary Group (IDG) meetings, participating in patient care planning, and collaborating with the IDG to promote coordinated patient care. Additionally, the Social Worker will provide individual patient/family counseling, crisis intervention, and assistance with advance directives, funeral planning, and legal/fiscal responsibilities. They will facilitate community resources based on patient/family needs, accept clinical assignments consistent with their education and competence, and perform on-call responsibilities. All visits in EMR/HCHB must be completed and synced the same day, and the Time Tracker should be reviewed daily with Non-Visit Activity (NVA) entered when applicable. The employee must abide by and demonstrate the company Mission and 12 Heart’s Desires, participate in all mandatory staff in-services and meetings, maintain compliance with internal annual education requirements, and submit all documentation in a timely manner. All business activities must be conducted professionally and ethically, with other duties performed as required. The role ensures overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
