This role involves participating in an interdisciplinary team to serve the Center's population, providing therapeutic intervention to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities, and their feelings about institutionalization and separation from family and community. The Social Worker will maintain written documentation in resident medical records, perform initial psychosocial needs assessments, and provide quarterly written documentation for all residents, with ongoing documentation for short-term residents. A key part of the role is developing the social service component of the resident's plan of care, identifying problems, goals, and approaches, and participating in interdisciplinary care planning conferences. The Social Worker will also ensure resident and family input into the care planning process, act as a liaison between residents, families, outside agencies, and the Center Administrator to uphold resident rights, conduct family meetings, facilitate discharge planning, and ensure the safety of all Center customers. Additionally, the role involves coordinating admissions through generating referrals, developing relationships with referral sources, verifying insurance coverage, and conducting facility tours.
