About The Position

Sharon Hospital has an opening for a PRN Social Worker in the Psych Unit. This role requires covering a minimum of two shifts per month. The Social Worker is responsible for providing social service and discharge planning functions for the inpatient behavioral health unit. This includes completing psychosocial assessments, contributing to the integrated treatment plan, running therapeutic groups, and coordinating discharge plans. The role also involves maintaining performance improvement activities within the department and participating in PI activities, serving the population based on the scope of services in the department.

Requirements

  • Master Social Worker (MSW).
  • Licensed Master Social Worker (LMSW).
  • Minimum of three (3) years’ experience in all aspects of social service and discharge planning.
  • Connecticut Licensed Clinical Social Worker (LCSW) or license eligible
  • Knowledge of Microsoft Office, including Word, Excel, PowerPoint and Outlook.
  • Basic computer knowledge.
  • Excellent customer service skills.
  • Ability to multitask, make decisions and communicate effectively.
  • Able to communicate effectively in English, both verbally and in writing.

Nice To Haves

  • Two (2) years of social work experience.
  • Bilingual – English/Spanish

Responsibilities

  • Responsible to perform psychosocial assessments within 72 hours of admission.
  • Identifies and addresses psychosocial needs of patient/ family and provides support, counseling and crisis intervention.
  • Ability to assist physicians and communicate, social service, post-hospital care, and discharge planning needs of the patient.
  • Leads insight-oriented, psychoeducational, process, or skills groups.
  • Runs family meetings with both the physician and patient as needed.
  • Provides support with advance directives and assists patient/family with the grieving process and makes referrals to Pastoral Care, as appropriate.
  • Accurately determines and communicates the type of assistance, setting, or resources necessary for the patient/family and provides appropriate resources.
  • Resolves personnel concerns at the departmental level, utilizing the appropriate processes as required.
  • Maintains safety and regulatory requirements, including all state, federal and Joint Commission regulations.
  • Documentation meets current standards and policies.
  • Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to patients when needed.
  • Acts on performance improvement issues identified by behavioral health leadership team.

  • Demonstrates regular, reliable and predictable attendance.
  • Performs other duties as required.
