This specific role is within the Acute Rehabilitation Unit (ARU) and involves providing comprehensive social work services to patients, families, and the community, in accordance with licensure and regulatory requirements. The social worker will coordinate and facilitate services for patients during and after their admission to the ARU, focusing on discharge planning, care coordination, and counseling to ensure a smooth transition through the continuum of care in a cost-effective manner. The position also offers opportunities to expand psychosocial support through groups and participation in interdisciplinary team meetings.

Requirements

Bachelor’s degree in social work
Effective communication skills, both verbal and written

Nice To Haves

Master’s degree in social work
Three years’ experience in acute care or other related healthcare field
Utilization Review/Management and/or Case Management Certification
Previous experience in case management and/or discharge planning

Responsibilities

Fulfills the needs of patients, families, and community by providing Social Work services in accordance with his/her licensure, certification, training, and regulatory requirements.
Assumes the responsibility for coordinating and facilitating services for patients during and subsequent to admission to the acute rehabilitation unit.
Conducts discharge planning, coordination of care, and other planning and counseling activities to effectively move the patient through the continuum of care in a cost-effective and resource efficient manner.
Responds effectively to new and changing work assignments.
Follows ARU patients upon admission to the unit (completing a thorough Unit Program Orientation and introduction to Social Work discharge planning role upon) through discharge.
Facilitates coordination of continued rehabilitation and after discharge, such as home health therapies, outpatient rehabilitation or continuing care centers (such as Long-Term Care Centers and Skilled Nursing Facilities).
Supports medical team(s) in coordination of community health care needs such as home nursing care, IV therapies, wound care, outpatient hemodialysis, etc.
Coordinates/orders durable medical equipment.
Supports patients and family to locate various community and insurance related benefits/resources.
Schedules and facilitates family meetings, as needed.
Keeps track of medical insurance benefits and authorization for stay in inpatient rehab, updating insurance providers- seeking additional authorization when it is needed.
Participates in 3x weekly interdisciplinary team meetings and collaborates with medical and therapy teams to support discharge planning process, including advocating for patient/family needs and preferences.
Provides bedside coping and adjustment support to patients and families.
Keeps patients and family members updated to discharge planning tasks and progress throughout rehab course.
Expands the unit’s psychosocial support with the re-introduction of a coping skills/adjustment group.
Participates in additional groups (Stroke and Amputee groups).
