Requirements

  • Bachelor’s degree in Social Work (BSW) or related field
  • Valid state licensure or certification as a Licensed Social Worker (LSW) or equivalent
  • Strong interpersonal and communication skills, with the ability to build rapport with diverse populations
  • Knowledge of healthcare regulations, Medicaid/Medicare policies, and community resources
  • Ability to handle sensitive situations with empathy, professionalism, and discretion
  • Excellent organizational and documentation skills
  • Ability to work collaboratively within a multidisciplinary team

Nice To Haves

  • Master’s degree preferred but not required
  • Proven experience working in a healthcare or skilled nursing environment experience preferred

Responsibilities

  • Conduct psychosocial assessments of residents to identify their needs, preferences, and potential barriers to care
  • Develop and implement individualized care plans in collaboration with residents, families, and healthcare teams
  • Provide emotional support to residents and their families to help them cope with health challenges and transitions
  • Coordinate with community resources, agencies, and support services to meet residents’ social and emotional needs
  • Advocate for residents’ rights and preferences within the healthcare setting
  • Document all interactions, assessments, and care plans accurately and in compliance with regulatory standards
  • Participate in interdisciplinary team meetings to ensure comprehensive resident care
  • Educate residents and families about available services, benefits, and community resources
