The Social Worker provides direct and indirect social worker services primarily on a visiting and intermittent basis in the patient’s home in accordance with the policies and procedures of Catholic Home Health Services. This role involves instructing, treating, observing, and evaluating patients with social and emotional problems affecting their health status, participating in the development and re-evaluation of the Plan of Care, and assessing pre-bereavement/anticipatory grief needs. The Social Worker implements the Plan of Care to meet patient needs, including social service goals, supportive counseling, problem-solving, and community referrals. They must also apply the agency’s infection control plan and universal precautions to protect patients and staff.
