The Social Worker provides an assessment for and provision of psychosocial services and support to patients and their families. This role involves performing initial and ongoing assessments of the patient and family's psychosocial status, assisting in accessing resources for social, financial, and community needs, and developing a plan of care to optimize the patient and family's ability to cope with the psychosocial impact of illness and impending death. The Social Worker offers support, guidance, and counseling, provides information about and refers to community resources, and educates patients and families about Advance Directives. They also initiate discussions about funeral planning and burial arrangements, participate in interdisciplinary care planning, and collaborate with team members on psychosocial issues. Additionally, the role includes assessing bereavement needs, developing bereavement plans of care, facilitating bereavement support groups, and conducting presentations to professional and community organizations. The Social Worker is responsible for attending department meetings, serving on committees, attending mandatory in-services, completing clinical documentation in a timely manner, notifying team members of significant changes in patient status, completing EMR documentation, and following agency protocols. Participation in the after-hours on-call program is also required.
