About The Position

Primary function is to provide coordinated care in the home or facility to patients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient/family. The Social Worker will assess hospice patients to identify psychosocial, grief, financial, environmental, and community needs. They will use effective interpersonal relations and communication skills to facilitate the use of these skills by other team members to achieve desirable outcomes. The role involves meeting mandatory continuing education requirements, maintaining a working knowledge of public and private eligibility standards and requirements, and attending Inter-Disciplinary Group (IDG) meetings to participate in patient care planning and promote coordination of care. The Social Worker will provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues, and transfer of responsibilities relative to legal and fiscal issues. They will facilitate community resources based on patient/family needs, accept clinical assignments consistent with education and competence, and perform on-call responsibilities. All visits are to be completed and synced same day in EMR/HCHB, and Time Tracker should be reviewed daily with Non-Visit Activity (NVA) entered when applicable. The employee must abide by and demonstrate the company Mission and 12 Heart's Desires through behavior and job performance, participate in all mandatory staff in-services and meetings, and maintain compliance with internal annual education requirements. Timely submission of all documentation and conducting all business activities in a professional and ethical manner are required. The role also includes ensuring overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.

Requirements

  • Bachelors or Master’s Degree in Social Work from a State accredited program.
  • Current State license required in accordance with State regulations
  • Current State Drivers License
  • One year of experience as a Social Worker in a clinical care setting required
  • Strong critical thinking, organizational, interpersonal, and communication skills
  • Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary group setting
  • Ability to work autonomously
  • Working knowledge of community resources
  • Must have reliable transportation and provide ongoing valid and current auto liability insurance

Nice To Haves

  • If employee has a Bachelor's degree and is hired after 12/02/08, must be supervised by a Master's of Social Work (§418.114).
  • hospice experience preferred
  • Experience in EMR a plus

Responsibilities

  • Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals
  • Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes
  • Meet mandatory continuing education requirements of the agency/licensing board
  • Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements
  • Attend Inter-Disciplinary Group (IDG) meeting, participate in the patient care planning process and collaborates with IDG to promote coordination of patient care
  • Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues
  • Facilitate community resources based on patient/family needs
  • Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned
  • In EMR/HCHB, all visits are to be completed and synced same day
