Social Worker
About The Position
This role involves providing essential support to patients and their families within a healthcare setting. The Social Worker will conduct psychosocial assessments to understand complex emotional, psychological, and physical issues, and collaborate with an interdisciplinary team to develop and reevaluate individualized care plans. A key aspect of the role is patient advocacy, ensuring patient rights and preferences are respected, particularly in sensitive situations like end-of-life decisions or cases of abuse and neglect. Additionally, the Social Worker will connect patients with necessary community resources for financial assistance, housing, food, and long-term care placement.
Requirements
- Bachelor's degree in Social Work required.
- 3 years of experience preferred.
Nice To Haves
- Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
- ACM also accepted.
- Certified Social Worker preferred.
Responsibilities
- Conducting psychosocial assessments to determine patient and family support needs.
- Collaborating with physicians, nurses, and therapists for interdisciplinary care planning.
- Developing and reevaluating individualized care plans from admission through discharge.
- Advocating for patients to ensure their rights and preferences are respected.
- Connecting patients with community resources for financial assistance, housing, food, and long-term care placement.
