The Social Worker is responsible for participating in an interdisciplinary team approach to better serve the needs of the Center population. They provide therapeutic intervention to help residents cope with the social and psychological aspects of their illnesses, impairments, or disabilities, as well as their feelings about institutionalization and separation from family, community, and loss. The role involves maintaining written documentation in the resident medical record, performing initial psychosocial needs assessments, and developing the social service component of the resident's plan of care. The Social Worker also participates in interdisciplinary care planning conferences, ensures resident and family input into the care planning process, and acts as a liaison between residents, families, outside agencies, and the Center Administrator. Additionally, the role facilitates discharge planning, coordinates admissions through generating referrals, develops relationships with referral sources, verifies insurance coverage, and conducts facility tours.
