As a hospice social worker, you provide emotional and logistical support to patients and their loved ones during end-of-life care. You help patients get their affairs in order in accordance with their wishes, including matters related to finances and funeral arrangements, and support loved ones as they navigate grief. You work with clinical teammates to develop comprehensive plans of care and deliver compassionate, personalized support that honors each patient’s wishes. Most of your time is spent caring for hospice patients wherever they call home, including private residences and assisted living or skilled nursing facilities. About once a week, you come to the office to meet with the interdisciplinary team, including RNs, physicians, chaplains, nurses, and aides.
