The Social Worker provides medically related social work services to support the physical, mental, and psychosocial well-being of each resident and their family, in accordance with Elizabeth Seton Children’s Center's philosophy, mission, standards, and policies. This role involves case management, psychosocial assessments, assistance with benefits and legal/social needs, and collaboration with an interdisciplinary team to ensure high-quality care. The Social Worker also advocates for residents' needs, works with the child welfare system, prepares residents for discharge, provides palliative and end-of-life services, and maintains documentation in the EMR and MDS.
