About The Position

The Social Worker is a key member of the Case Management team and is responsible for assessing the social, emotional, financial, and environmental needs of patients and their families to support safe, effective care transitions. This role collaborates closely with physicians, nurses, case managers, and community partners to develop and coordinate individualized discharge plans, connect patients with appropriate resources, and help address barriers to recovery and ongoing care. The Social Worker provides counseling, crisis intervention, patient advocacy, and education while ensuring compliance with regulatory requirements and organizational standards. This position requires strong critical thinking, sound judgment, and the ability to work independently in a fast-paced acute care hospital environment.

Requirements

  • Medical Social Work experience in an acute care or healthcare setting is required.
  • Master of Social Work degree
  • Board Certified Social Worker, or related field (i.e. counseling) in the state of North Carolina
  • Handle With Care within 30 days of hire
  • Basic Life Support certification
