About The Position

Benton County is actively recruiting for a full-time Social Worker position within the Human Services Department, specifically in the Child Protection/Intake/Assessments Unit. This role focuses on child protection/children's services and family assessment duties, along with related responsibilities to support the safety and well-being of children and families in the community. The Social Worker’s primary purpose is to ensure the health and safety of individuals we serve while providing person-centered planning aimed at enhancing their quality of life, increasing happiness, promoting independence, and supporting autonomy. In this role, the Social Worker must determine eligibility for county-coordinated and monitored programs and support clients in making informed decisions about available services. Eligibility determination includes, but is not limited to, conducting required assessments and collecting financial and other relevant information necessary to complete the process. This Child Protection/Child Services Case Management position may require work outside of normal business hours. The primary caseload for this Social Worker position will focus on Child Protection Family Assessments. This is a full-time position with a standard departmental schedule of 8:00 a.m. to 4:30 p.m., Monday through Friday. This position is required to participate in the on-call rotation for approximately two weeks per year. Additional compensation is provided for on-call assignments.

Requirements

Bachelor's degree in social work, psychology, sociology, or a related field.
Must possess a valid driver's license and be able to drive in the state of Minnesota.
Must successfully complete a background check.
Obtain and maintain certifications and licensures.
Ability to maintain, review, and interpret program management reports.
Knowledge of federal, state, and local laws and regulations, including data privacy and human services programs.
Understanding of basic social work practices and a working knowledge of other units and services within the agency and community resources.

Must complete Child Protection Foundations Training within six months of hire.
Must complete a minimum of 15 additional hours of continuing education annually.
Must complete Child and Restraint Systems (CARS) Training before transporting children.
Must complete Forensic Interviewing Training within six months of hire (CPS Investigations).
Must demonstrate the ability to communicate effectively in both written and oral formats.
Must complete Minnesota Department of Human Services (DHS) Foundations Training within six months of hire.
Must complete Minnesota Department of Human Services (DHS) Permanency Training within one (1) year of hire.
Must complete the Permanency and Adoption Competency Certification (PACC) Training within two (2) years of hire.
Must complete a minimum of 15 hours of continuing education annually.
Must obtain Rule 79 Certification within one (1) year of hire.
Must complete a minimum of 15 hours of mental health training annually.
Must complete Fire Marshall inspection training every four (4) years.
Must complete Minnesota Department of Human Services (DHS)-required licensor training within the first six months of employment.
Licensors must complete a minimum of six (6) hours of licensing-related training annually.
Adult Protection training must be completed within one (1) year of hire, followed by a minimum of eight (8) hours of continuing Adult Protection training annually.
Must complete the Minnesota Department of Human Services (DHS) Person-Centered Support Plan Training within one (1) year of hire.
Must complete a minimum of twenty (20) hours of annual training in case management or developmental disabilities.
Required education and training must include: person-centered planning, informed choice, cultural competency, employment planning, community living planning, self-direction options, use of technology support, and Getting to Work: A Case Manager's Guide to Supporting Employment.
Must have a minimum of one (1) year of experience in the education or treatment of individuals with developmental or related conditions, or completion of at least one course specifically focused on developmental disabilities.
MnCHOICES Certification must be completed within six months of hire.
Maintain MnCHOICES Certification by completing recertification requirements every three (3) years.
Knowledge of Maxis, MMIS, SMRT, and MnCHOICES Revised Tool.

Responsibilities

Assess clients to determine individual needs and establish treatment/service goals.
Maintain caseload and provide ongoing case management, including crisis management.
Effectively interview and engage clients to assess and evaluate needs.

Write up child protection intake reports from law enforcement, hospitals, educators, and other mandated reporters.
Check MGA, Maxis, and SSIS for history regarding new intakes.
Take Behavioral Health Fund (BHF) and Commitment requests.
Write up child and adult mental health intakes.
Screen to see if reports meet maltreatment guidelines. Intake is responsible for screening out reports, cross-reporting to law enforcement, and sending mandated reporter letters.
Provide community resources to residents.
Perform duties for adult and child mental health-targeted case management. Adult duties include case management for voluntary and civilly committed clients.
Coordinate with mental health, housing, medical, educational, and vocational providers.
Monitor client compliance with terms of court orders, provisional discharges, and report to the courts, Minnesota Department of Human Services (DHS), and State Forensics Services to ensure the safety of the client and community members.
Create strategies to improve and monitor attendance to avoid court action.

Provide support with investigations into complaints and violations of licensing statutes.
Ensure MN statute and rules governing licensed foster care.
Assist providers through the initial licensing process and investigate complaints and violations of licensing statutes.
Coordinate visits and inspections to maintain licenses.
Complete investigations of foster care and family childcare sites.
Follow up with the Minnesota Department of Human Services (DHS) to ensure procedures are followed to implement consequences for licensing violations.
Coordinate and implement strategies for recruitment and appreciation of foster providers and facilitation of support meetings.
Licensors participate and/or facilitate regional and state resource/consultation meetings.
Monitor cases and review for Rule 13 compliance by the Minnesota Department of Human Services (DHS).
Coordinate and complete unannounced visits on an annual basis to maintain licenses in childcare, use the ELICI checklist, and ensure compliance with rules.
Protect and promote safety, independence, and quality of life for vulnerable adults.
Assist in providing safe environments for adults.
Petition the court to establish emergency and full guardianship/conservatorship as directed by the courts/judicial district.
Conduct Adult Protection Investigation as assigned.

Coordinate services for Community First Services and Supports recipients.
Waiver case management services to individuals who are eligible for Alternative Care, Elderly Waiver, Community Alternative Care, Community Access for Disability Inclusion waiver, Developmental Disability Waiver, Rule 185, Community First Services and Supports, Family Support Grant, Semi-Independent Living Services, Relocation Services, and Essential Community Support.
Evaluate and determine services needed to support health and safety for individuals with disabilities or the aging population.
Educate individuals receiving services, family members, and community providers of services available through waiver programs.
Develop a person-centered care plan.
Coordinate referrals and services. Assist with access and navigation of social, health, educational, vocational, housing, and other community support and services.
Manage individual service plan budgets within state guidelines and cost-effectiveness.
Monitor the quality and effectiveness of services provided through waiver funding for the safety and well-being of the person receiving the services.
Develop, revise, and update internal agency policies and procedures for Waiver Case Management as assigned.
Determine individual needs and eligibility for a multitude of county, state, and federal programs through MnCHOICES assessments.
Take on projects assigned to develop, revise, and update policies and procedures.
Attend MnCHOICES meetings with local counties.
Prepare for, respond to, and represent Benton County at court appeal hearings.
Complete all paperwork included with the MnCHOICES Assessment.
Complete the residential service plan if the person is living in a customized living setting and will be accessing Elderly Waiver (EW).
Determine an individual’s needs and eligibility for eleven (11) different counties, state, and federal programs.
Provide professional, accurate, and timely assessments for clients to determine individual needs.
Connect chronically homeless individuals with housing and available housing streams to support housing.
Develop and maintain a community housing resource list.
Develop relationships with community property owners to assist in increasing housing options for hard-to-place individuals.
Screen for basic needs and assist with referral services to community living resources that meet individual client needs and choices.
Assist clients with the application process to secure housing.
Work collaboratively in several multi-disciplinary teams such as BCAT, CAMHI, and other housing-related groups.
Provide information and referral services to individuals from diverse populations both in the jail and in the community for housing, mental health, and chemical health issues.
Complete jail duties as assigned.
Respond to Mental Health calls as requested.
Provide daily needs supplies to clients.
Coordinate point in time (PIT) count.
Data entry as required by grants.
Other duties as assigned or apparent.
