Requirements
- Current License under the Social Workers, Marriage and Family therapists and Professional Counselors Act, or a Bachelors, masters or doctoral degree from an accredited school or program in social work or social welfare.
- 1 year experience in health or related field.
- Strong written and verbal communications skills.
- Working knowledge of computers, Microsoft Word and Excel required.
Responsibilities
- Serves as an advocate to the Resident in a person center directed culture.
- Is a part of the interdisciplinary team, responsible for identifying and developing care plans that address the psycho-social needs of the resident/patient.
- For assigned residents/patients, insure appropriate care plans.
- Acts as resident/patient advocate.
- Provides staff education as appropriate.
- Develops goals for residents/patients and works with residents/patients to achieve these goals.
- Acts as liaison and provides educational support to residents/patients and their families.
