About The Position

Provide social work services to patients in acute care, maternal child care, emergency department, home health/ hospice setting for patients with acute, chronic, or terminal illness and their families. Services to patient and families include: identification of psychosocial needs, develops, and implements plan of care from the strengths perspective responsive to patient/family concerns, health care status, available resources, age specific problems and clinical impressions; discharge planning, crisis intervention, information and referral, protective services, patient family education, collaboration with the multidisciplinary care team, and building connections with community resources. Requirements

  • Bachelor degree Social Work
  • Licensed Bachelor of Social Work
