This role provides supportive services and counseling on healthcare and home care programs and services. The Social Worker serves as a member of the interdisciplinary team, assisting with social, emotional, and economic concerns of patients, clients, and residents, and their families or caregivers. The goal is to enable them to achieve or maintain an optimal level of functioning by coordinating and planning programs. This includes providing crisis intervention, assisting families in understanding medical situations, and coordinating healthcare programs among patients, families, and healthcare teams. The role also involves responding to suspected abuse, neglect, or violence, developing appropriate plans of care, and providing referrals and information about available programs and services. Demonstrating professionalism through care conferences and transitional rounding, advocating for patients, and utilizing time effectively are key aspects of the position. The role requires critical thinking, problem-solving, decision-making, and strong communication skills to establish rapport with various stakeholders. Depending on the department, social services may be provided for donors and transplant recipients.
