About The Position

Brookings Health System is seeking a Social Worker to join their team. This position is approximately 20 hours per week and is benefit eligible. The Social Worker is responsible for psycho-social assessment and plan of care, psycho-social interventions, and is a member of the interdisciplinary hospice team. This role involves maintaining open communication and providing consultation among families, patients, tenants, social services designee, hospital discharge planner, and nursing staff. Responsibilities include performing assessments related to suicidal ideation, abuse and neglect, safety, ability to live independently, and mental health. Additionally, this position includes coordinating the hospice bereavement program and serving as the hospice volunteer coordinator.

Requirements

Holds a bachelor (or related field) or master's degree, at a minimum, in social work
Supportive of the Hospice concept and demonstrates understanding of the needs of the terminally ill person, family and significant others
