Requirements

BS in Social Work degree required.
Social Work certification to practice in NYC.
1-2 years of experience working with Geriatric population.
Nursing & Rehabilitation experience.
New graduates with experience are welcomed.

Responsibilities

Assessing, counseling, interviewing, and educating clients and their family members.
Assisting in planning, organizing, and implementing established facility policies and procedures to meet the medically related emotional and social needs of the patient/resident.
Assessing the nature of patients' situations by interviewing and reviewing personal history.
Planning, coordinating, managing, and implementing support packages to help patients/residents deal with difficulties and overcome dependencies.
Developing and executing individual treatment plans.
Applying for Medicaid and Medicare for patients.
Assisting patients with Food Stamps, Housing applications, and phone assistance.
Interviewing service users and assessing their current condition, needs, strengths, and weaknesses.
Addressing each case as a unit and setting tailored, measurable goals.
Monitoring and evaluating patients’ progress and modifying treatment plans accordingly.
Offering information and counseling on the best course of action during sessions.
Facilitating referrals to other healthcare professionals and programs.
Maintaining accurate patient documentation.
Acting as a key-worker and cooperating with multidisciplinary teams.
Supporting the Facility.
Being knowledgeable of patient/resident rights and promoting an atmosphere that allows for the privacy, dignity, and well-being of all residents in a safe, secure environment.
Supporting and participating in teamwork.
Complying with the Code of Conduct.
