In this role, you will support families of children undergoing new tracheostomy placement by facilitating program enrollment, providing psychosocial support, and coordinating post-discharge follow-up. This role will involve active participation in interdisciplinary rounds, family engagement prior to and preparation for discharge, structured follow-up after hospitalization, and collaboration with clinical and grant teams to ensure high enrollment and program fidelity. You will also serve as a valuable resource as you communicate with patients, their families and the highly skilled interdisciplinary team to assure excellent outcomes and an outstanding patient experience. Requirements
- Master’s Degree in Social Work (MSW)
- New York State License as a Licensed Masters Social Worker (LMSW)
Nice To Haves
- At least one year of inpatient pediatric experience.
Responsibilities
