About The Position

HopeWest is seeking a Social Worker to support patients and families in Delta County. This PRN/Variable role offers flexible scheduling and involves working in a "mobile office" environment, visiting patients and families in their homes within the Delta County community. The Social Worker will be part of an interdisciplinary care team, collaborating with social workers, chaplains, nurses, physicians, and certified nursing assistants to enhance the lives of those in need. Key responsibilities include acting as an advocate, providing education and resources, conducting initial and ongoing assessments of needs, developing Plans of Care, assisting in the integration of volunteers into patient care teams, and performing initial and ongoing assessments of bereavement risk levels for families and friends.

Requirements

  • Bachelor's degree from an accredited social work program required (Master's degree preferred).
  • If Master's prepared, obtain and maintain Colorado licensure as a Licensed Social Worker (LSW) or Clinical Social Worker (CSW) with the State of Colorado Department of Regulatory Agencies (DORA). If not licensed at time of hire, must obtain within 90 days.
  • Minimum of one (1) year supervised experience in the health care field or during a supervised internship.
  • Valid Colorado driver's license.
  • Adequate auto insurance.
  • Unlimited access to reliable transportation maintained throughout employment.
  • Able to work in a variety of environments.
  • Computer proficiency in Microsoft products (Word, Excel, and Outlook) and EMR.
  • Ability to cope with patient/family emotional stress within the framework of the hospice philosophy.
  • Ability to function as an effective member of the interdisciplinary team.
  • Effective communication, problem-solving, and customer relation skills.
  • Ability to interact appropriately with geriatric populations and clients with cognitive behavior impairment and other various barriers to communication.
  • Demonstrates a driven curiosity for knowledge of available community resources for patients/families.
  • Current CPR certification maintained throughout employment.

Responsibilities

  • Provide social work services, including counseling and group facilitation, for assigned patients and families.
  • Conduct initial and ongoing assessments of patient and family needs.
  • Develop and maintain a Plan of Care in collaboration with the interdisciplinary team.
  • Play a key role in adding volunteers to the patient's care team.
  • Perform initial assessment of bereavement risk levels for family and friends, and re-evaluate as needed.
  • Act as an advocate by providing education and resources to patients and their families.
  • Participate in on-call, holiday, weekend, and after-hours rotation.
