About The Position

DaVita is looking for a PER DIEM Licensed Social Worker (LISW) (LICSW) (LGSW) to support our patients and their families at our Cass Lake Dialysis Clinic in Cass Lake, MN! Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease. Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting. Requirements

• Licensed or registered in the state of practice as required by state regulations -- (LISW) (LICSW) (LGSW)
• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

Responsibilities

• Covering Cass Lake Dialysis clinic only
• Will need to be in clinic at least twice a month
• Provide group supervision 4 hours a month
• Support and advocate for patients transitioning to life on dialysis
• Help patients understand their rights and responsibilities
• Guide patients in managing the physical, mental, emotional and financial demands of End Stage Renal Disease
• Build meaningful and long-term relationships with patients and their families
