The Acute Care based LMSW Social Work Case Manager provides advance clinical social work services which may include crisis counseling, support group development and execution, care planning, case management, and community resource coordination to patients and their families who utilize outpatient neuro program services at Sunnyview Rehabilitation hospital. Work involves the use of sophisticated interviewing, assessment, and multi-modality psychotherapeutic interventions aimed at positively affecting the physical, cognitive, and emotional functioning of patients and their families. Work involves use of high-level creative problem solving and advance communication skills while performing under mainly autonomous conditions. The master’s level social work case manager identifies barriers which impact optimal patient functioning and ensures patient receives appropriate resources in most appropriate setting.
Requirements
- Master’s Degree in Social Work Required.
- New York State License required.
- At least one-year previous experience in Medical, Psychiatric, or Health Care settings required.
Responsibilities

- Provide specialized skills and mental health therapy for clients.
- Identify barriers which impact optimal patient functioning
