About The Position

The Social Worker assesses the mental and emotional functioning of residents and administers services for necessary treatment. This role provides clinical supervision to Case Managers through case analysis, service planning, and supervisory processes. The Social Worker also provides direct treatment services and issues referrals on an as-needed basis.

Requirements

  • Master’s degree in social work required.
  • Bi-lingual, Mandarin or Spanish preferred.
  • A minimum of three (3) years’ experience working with homeless population.
  • Demonstrated sensitivity and experience with and/or knowledge of homelessness within disenfranchised communities including individuals who identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, the elderly and individuals with substance abuse and/or mental health issues.
  • Familiarity with DHS regulations and DHS Cares Reporting.
  • Must have excellent communication skills.
  • Ability to write and record case documentation.
  • Skills in group counseling.
  • Clinical experience to include the ability to make psychosocial assessments and develop and implement individual living plans.
  • Ability to work in collaboration with other departments, agencies, systems, and institutions to maximize scarce resources.
  • Must be computer-literate and must have knowledge of Microsoft Office applications.
  • Ability to work a varied schedule.
  • Will be required to read printed and handwritten materials.
  • Will be required to use computer mouse and keyboard.
  • Will be required to read information on computer screen.
  • Must be able to sit for extended periods of time.
  • Must be able to enter information in digital forms and spreadsheets.

Responsibilities

  • Assess and evaluate existing, new, and prospective clients/families referred by NYC Department of Homeless Services.
  • Develop individual living plans (ILPs) for families, including all adults and children, and provide therapeutic support as needed.
  • Provide clinical supervision to Case Managers.
  • Conduct clinical case conferences with clients in collaboration with internal staff and external providers to ensure compliance with individual living plan goals and address unmet needs.
  • Participate in clinical caseload conferences and staff meetings.
  • Responsible for crisis intervention, including on-call duties via agency phone as needed.
  • Attend trainings and consortium meetings (e.g., with DHS and others) to discuss issues pertaining to service delivery.
  • Evaluate mental health status for residents upon admission by completing comprehensive evaluations (CFEs).
  • Coordinate mental health/psychiatric care and referrals.
  • Develop a resource network of social services providers.
  • Develop and facilitate group meetings.
  • Monitor resident records for compliance and best practices.
  • Function independently during facility coverage, including taking appropriate action in crisis situations per agency procedures.
  • Regularly assess the safety and well-being of all clients.
  • Participate in weekly individual and group supervision with the Social Work Supervisor.
  • Participate in training seminars to enhance and develop clinical skills.
  • Assist with reports and program evaluation.
  • Review progress notes and service plans.
  • Monitor client compliance.
  • Develop strategies for engaging individuals with trauma histories.
  • Perform other duties as assigned by the Social Work Supervisor.
