Social Worker – Center for Transgender Medicine & Surgery (CTMS)
About The Position
Social Work Services at the Mount Sinai Hospital is seeking a Social Worker for the Center for Transgender Medicine & Surgery (CTMS). The Mount Sinai Hospital is a large, comprehensive facility with an extensive outpatient and specialty care network. Our Social Work department, founded in 1907, is one of the oldest in the nation and employs over 450 licensed social workers across numerous program areas. We offer a pathway to LCSW licensure, continuing education, and professional development. This role utilizes a multidisciplinary team approach to provide coordinated care to trans-identified patients seeking gender-affirming medical interventions, including surgeries. The team collaborates to support patients through facial feminization, chest reconstruction, and genital reconstruction surgeries.
Requirements
- Experience working with LGBTQIA+ individuals with specific familiarity with the transgender individuals.
- Excellent engagement, organizational, collaborative, and clinical skills.
- Excellent administrative and time-management skills.
- A collaborative and flexible attitude to meet the changing needs of the interdisciplinary team.

Responsibilities
- Complete full psychosocial assessments.
- Work closely with the CTMS interdisciplinary team (nurse coordinator, eligibility assistant, mental health professionals, primary care providers, surgeons, medical specialists, and outside service providers) to support trans patients through their medical transitions.
- Facilitate appropriate medical interventions when required.
- Provide surgical support, including confirming mental health evaluations, necessary letters, pre-surgical treatments, and determination of post-surgical community resources.
- Collaborate with inpatient social workers, discharge planners, and outside agencies.
- Develop and maintain support groups for CTMS patients (pre- and post-surgical).
- Provide short-term therapy (pre- and post-op).
- Participate in CTMS team meetings, steering committee meetings, and community advisory board meetings.
- Perform administrative duties such as creating and organizing patient lists with up-to-date surgical information.
- Co-lead case conference meetings to discuss high-risk patient issues.
- Mediate between in-patient and out-patient teams to facilitate smooth and comprehensive care throughout the surgical process.
