About The Position

The social worker acts as a patient advocate to the hospital clients. This is an autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost-efficient patient outcomes. The role involves determining medical and social services eligibility for new admissions, interviewing clients to assess their social situation, and identifying psycho-social problems that could interfere with the treatment plan. The social worker provides supportive counseling to clients, families, and staff, helping them understand the client's physical and/or mental disability, and encouraging active participation in the treatment plan. Responsibilities include conducting home visits for follow-ups or assessments, coordinating with other agencies and organizations for client services, and collaborating with interdisciplinary staff and external organizations. The position also participates in quality improvement and evaluation processes related to patient care management. The social worker is on-site and available seven days a week, including holidays, and is required to work an on-call rotation schedule.

Requirements

  • Current Healthcare Provider BLS Certification

  • Master’s degree in Social Work
  • Graduate from an accredited school
  • Two (2) years of Social Work experience
  • Experience in a hospital setting
