Requirements

Bachelor’s Degree in Social Work or related human services field such as sociology, gerontology, psychology, rehabilitation counseling, or special education
One year of supervised social work experience working directly with individuals in long term care or rehabilitation
Strong communication and interpersonal skills
Ability to think critically and solve problems
Someone who genuinely cares about improving the lives of older adults

Nice To Haves

Michigan Social Work license preferred

Responsibilities

Conduct psychosocial assessments to identify resident needs and concerns
Support residents and families with counseling, adjustment to care, and emotional support
Participate in interdisciplinary care plan meetings
Advocate for resident rights, dignity, and quality of life
Assist with discharge planning and coordination of community resources
Work closely with nurses, therapists, physicians, and other team members
Lead resident and family meetings when needed
Maintain documentation in accordance with state and federal guidelines
Provide guidance to staff regarding psychosocial concerns
