About The Position

Under the supervision of the Director of Specialized Health and Social Services, and with oversight by the Regional Team Leads/Supervisors, provides advanced and comprehensive case management consultation for families with children who have health risks or special health care needs. Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. Oversees case management services for families in the Children with Special Health Care Needs Services Program, those that are on the CSHCN Program/Waiting list and non-Medicaid eligible, pregnant women on Title V or unfunded, children on the CHIP and CHIP Perinatal program, children referred by the Newborn Screening program, children referred by the Childhood Lead Poison Prevention program, children with a lapse in Managed Care Organization or Medicaid coverage/dis-enrolled, children carved out of the MCO service delivery model, children that are on traditional Medicaid, STAR Health clients remaining in Personal Care Services Program (PCS), and Children referred from the MCO’s who are in need of school advocacy, clients on the Personal Care Services (PCS) program, Department of Family and Protective Services (DFPS) and Case Management for Children and Pregnant Women (CPW) programs Assists families with eligibility determination for state services and conducts comprehensive assessments to determine the need for PCS/PDN, CSHCN, and CPW services. Travels to conduct home visits. Completes Intakes, Family Needs Assessments, Service Plans, and follow-up to families. Links families to community resources. Communicates program services in a positive and accurate manner. Works as a team member. Participates and advocates in Community Resource Coordination Groups (CRCG), case management coalitions, interagency groups and community organizations. Provides information and referral services. Completes administrative duties, monthly reports, special assignments and work activities in a professional and timely manner. Employee participates and/or serves in a supporting role to meet the agency’s obligation for disaster response and/or recovery or Continuity of Operations (COOP) activation; such participation may require an alternate shift pattern assignment and/or location. Works under minimum supervision with considerable latitude for the use of initiative and independent judgment. Maintains confidentiality.

Requirements

  • Must be a current unrestricted Licensed Social Worker in the State of Texas. (LBSW, LMSW, LMSW-AP, or LCSW)
  • Must provide own transportation and possess a valid Class C Texas Driver's license or equivalent license from another state.
  • Excellent written communication as observed by detail and completeness of information provided on the State Application
  • Willing to travel minimum of 65%

Responsibilities

  • Provides complex and specialized social work consultation and comprehensive case management services to families who receive services or are seeking services from the CSHCN-SP, children carved out of the MCO service delivery model, traditional Medicaid and STAR health clients remaining in PCS, SSI, NBS, CLPPP, CHIP, CHIP Perinatal, Pregnant women on Title V or unfunded, Department of Family and Protective Services, Personal Care Services Program, and CPW programs.
  • Conducts home visits for comprehensive assessment of home environment and needs.
  • Following program guidelines, provides comprehensive case management services including the Intake, Family Needs Assessment, Service Plan, Personal Care Services Assessment form, and follow-up services.
  • Assists families with the eligibility determination process for CSHCN-SP, and Personal Care Services Programs.
  • Provides information and referral services, linkage to community resources, family supports, advocacy, and service coordination.
  • Maintains documentation of work activities with record documentation, monthly reports, travel reports, case summaries and correspondence.
  • Utilizes software programs to do database entry, word processing, spreadsheets, electronic mail, and Internet.
  • Conducts comprehensive assessments to determine eligibility and need for PCS/PDN and Case Management Services.
  • Conducts assessments during home visits with families.
  • Documents identified needs and forwards documentation for authorization of personal care services.
  • Conducts reassessments for PCS/PDN, and Case Management as required or as necessary according to family needs.
  • Enters family encounter information into an online database.
  • Participates and collaborates with other community entities on health, social work, case management and other related issues.
  • Serves as the representative for the Texas Department of State Health Services (DSHS) on the CRCG, coalitions, interagency groups and advocacy groups to collaborate efforts in serving children with special health care needs.
  • Participates in the delivery of community disaster mental health services.
  • Maintains social service directory of community resources for service area.
  • Provides educational program presentations to the public, social service agencies, and HHSC programs to promote awareness of services available to children with health risks or special health care needs.
  • Attends training and conferences relevant to job to meet licensing requirement.
  • Continues to develop professional knowledge and skill in the practice of Social Work/Case Management in the Public Health setting.
  • Maintains Social Work Licensure.
  • Participates in performance evaluations process.
  • Attends staff meetings.
  • Performs other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation: such participation may require an alternate shift pattern assignment and/or location.
