About The Position

The Philadelphia Model for addressing Serious Mental Illness (SMI) in long term care provides highly structured therapeutic residential behavioral healthcare for frail adults who require skilled nursing level care and behavioral health services. Individuals served may have complex psychiatric needs and have reached maximum benefit from other community or hospital based mental health resources. Our program philosophy is centered on creating a welcoming, safe, and supportive environment where older adults with serious mental illness and complex behavioral health needs can receive care free of stigma. A multidisciplinary team delivers integrated long term care and behavioral health services. Requirements

  • Master’s degree in Social Work (MSW) required
  • Ability to manage a caseload in a fast paced, fully onsite environment
  • Proficiency with Electronic Health Records (EHR)
  • Full COVID 19 vaccination, where permitted by law

Nice To Haves

  • Professional social work licensure preferred but not required
  • Experience working with individuals with serious mental illness (SMI) strongly preferred
  • Experience in behavioral health, mental health, long term care, or human services settings preferred
  • Strong knowledge of community resources and social service systems
  • Excellent clinical documentation and organizational skills
  • Strong communication and interdisciplinary collaboration skills

Responsibilities

  • Conduct comprehensive psychosocial assessments
  • Participate in individualized treatment planning and care coordination
  • Provide social work services and ongoing case management
  • Link clients to community resources including healthcare, benefits, housing, and social services
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR)
  • Participate in interdisciplinary team meetings and clinical case reviews
  • Support discharge planning and continuity of care
  • Ensure compliance with HIPAA, confidentiality standards, and regulatory requirements
  • Collaborate with internal and external providers to support client stability and recovery
