Social Work Services at the Mount Sinai Hospital is a large department with over 450 licensed social workers across more than 65 program/service areas. This specific position involves providing social work services to cardiology patients in the Heart Hospital, with responsibilities including coverage of patients on 7C. There is a strong emphasis on complex discharge planning, requiring coordination of home care services and facilitation of placements. Coverage responsibilities will extend to other cardiac units.
