About The Position

Social Work Services at the Mount Sinai Hospital is a large department with over 450 licensed social workers across more than 65 program/service areas. Founded in 1907, it is one of the oldest hospital social work departments in the nation. The department believes that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level, including direct care and prevention. Employment in the department provides a pathway to LCSW licensure and access to continuing education and professional development opportunities. This specific position involves providing social work services to cardiology patients in the Heart Hospital, with responsibilities including coverage of patients on 7C. The social worker will be responsible for providing psychosocial assessments and services, helping patients and families cope with hospitalization and illness. There is a strong emphasis on complex discharge planning, requiring coordination of home care services and facilitation of placements. Coverage responsibilities will extend to other cardiac units.

Requirements

  • NYS LMSW or LCSW.
  • Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast-paced environment.
  • Skills in assessment and coordination of resources within hospital and community networks for effective management of patient care.
  • Excellent organizational, collaborative, and psychosocial assessment skills.
  • Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care.
  • Minimum one year in current position, unless currently a per diem or a float in Cardiac.

  • Inpatient hospital experience preferred.

Responsibilities

  • Provide psychosocial assessments and psychosocial services for patients.
