About The Position

Family Service League is seeking a full-time Social Worker for the Assertive Community Treatment (ACT) program. The Social Worker delivers services to individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches. The Social Worker will be given an agency vehicle to use for home visits with the clients. Travel is required.

Requirements

Master’s Degree required.
LMSW, LCSW required
1 year of experience working with the severe and persistent mental illness (SPMI) population required.
Ability to work independently and as part of a multi-disciplinary team required.
Excellent interpersonal and communication skills, both verbal and written required.
Proficient computer skills, including Microsoft Office and Electronic Health Records/Documentation, required.
Ability to multitask and prioritize in a fast-paced environment.
Valid and clean New York State Driver’s License required.

Responsibilities

Educate the clients and their families about how to manage a severe mental illness and meet their individual goals.
Conduct daily home visits using the agency vehicle.
Create individual treatment plans based on the clients’ needs.
Complete client assessments, relapse prevention plans, and housing applications.
Develop individual goals to work toward including a stable plan for medication, housing, and employment for each client.
Provide evidenced based practice treatment to clients with a severe and persistent mental illness.
Connect clients to treatment facilities including our clinics and other mental health resources in the community.
Perform crisis intervention by using the crisis phone after hours and on weekends, as needed.
Conduct group meetings with clients and their families to help rebuild trust and a relationship.
Travel is required using the agency vehicle.
All other duties as assigned.
