About The Position

Liquid Personnel is recruiting a Qualified Social Worker for its client’s Home First Team based in Norfolk. This is a highly rewarding opportunity to work within a fast-paced, multi-agency environment focused on safe and timely hospital discharges. Operating under a Home First ethos, the team is committed to promoting independence, recovery and person-centred outcomes by ensuring individuals receive the right care and support at the right time. You will work closely with health professionals, patients and their families, both within the hospital setting and in the community, carrying out assessments and reviews to support individuals transitioning back home or into appropriate care settings.

Requirements

  • Professional Qualification: MA, BA or BSc in Social Work, Diploma in Social Work or CQSW
  • Registration: Must be registered with Social Work England
  • Minimum of 2+ years’ post-qualified experience (including ASYE completion)
  • Experience working within Adult Social Care or hospital discharge settings
  • Proven experience completing assessments, support planning and reviews
  • Experience of multi-disciplinary and partnership working
  • Experience completing Mental Capacity Assessments and best interest decisions
  • Strong assessment and analytical skills within fast-paced environments
  • Ability to manage and prioritise a demanding caseload effectively
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively within a multi-disciplinary team
  • Confident decision-making and sound professional judgement
  • High standard of written work and case recording
  • Ability to build positive relationships with individuals, families and professionals
  • Strong organisational skills with the ability to meet deadlines
  • Commitment to person-centred, strengths-based practice
  • Strong working knowledge of: Care Act 2014, Mental Capacity Act 2005 (MCA), Safeguarding Adults frameworks and procedures

Nice To Haves

  • Experience supporting hospital discharges or working within a Home First/reablement model is highly desirable
  • Exposure to Continuing Healthcare (CHC) processes and reviews

Responsibilities

  • Managing a varied social work caseload within a hospital discharge setting
  • Undertaking person-centred assessments with patients and their families on the ward
  • Developing and implementing strengths-based care and support plans that promote independence and wellbeing
  • Supporting safe, timely and effective hospital discharges in line with the Home First model
  • Completing post-discharge reviews in individuals’ homes or care settings
  • Working collaboratively with multi-disciplinary teams, including health and social care professionals
  • Supporting individuals to access appropriate services, care packages and community resources
  • Undertaking Mental Capacity Assessments and making best interest decisions where required
  • Contributing to Continuing Healthcare (CHC) processes and reviews
  • Managing risk and safeguarding concerns in line with statutory responsibilities
  • Maintaining accurate, high-quality case records and documentation
  • Applying relevant legislation and best practice in all aspects of the role.
