About The Position

This position will work primarily with our after school Youth Intensive Outpatient Program (IOP) but may be asked to help out with our other child, adolescent and adult programs as well. The hours for this role are Monday - Thursday, 8a - 5p, with Fridays off.

Requirements

  • Master’s Degree in Social Work and licensure in the State of Minnesota required (LGSW).
  • Proficient in Microsoft Office Products including Word and Outlook.
  • Able to effectively and efficiently communicate in written and verbal form.
  • Ability to smoothly and rapidly shift between dissimilar tasks.
  • Ability to communicate well both verbally and in writing.
  • Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations.
  • Ability to make effective decisions while in stressful situations and circumstances.

Nice To Haves

  • Experience in healthcare setting preferred.
  • Experience using electronic health records preferred.

Responsibilities

  • Have knowledge and understanding of Social Work practices and principles.
  • Perform effective case psychosocial assessment and suicide risk assessment, discharge planning, and lead psychoeducational and/or therapeutic groups.
  • Develop treatment plans, complete discharge plans, and conduct discharge meetings.
  • Communicate with patients and families to keep them involved in treatment plan decisions.
  • Coordinate care with outpatient providers upon admission and discharge.
  • Facilitate weekly treatment team meetings and create master treatment plans/update treatment plans as agreed upon by the treatment team.
  • Contribute clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
  • Communicate clearly and effectively with clinical team members and Directors.
  • Be oriented and educated in gender, cultural differences, and age-appropriate care for the assigned department.
  • Manage disruptive and explosive patients after training.
