About The Position

An exciting opportunity is available to join the Country Health Connect team, where you will deliver high-quality, trauma-informed social work services through comprehensive psychosocial assessments, therapeutic interventions and person-centred care. The successful applicants will provide services across a range of programs throughout the Limestone Coast, with opportunities to work in Subacute Inpatient Rehabilitation, Acute Care, Aged Care and Childhood Health. Working as part of a supportive multidisciplinary team, you will help individuals, families and carers navigate complex health and social challenges while improving health outcomes and wellbeing. Full-time and part-time positions are available.

Requirements

  • A recognised qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
  • Experience providing comprehensive psychosocial assessment, counselling and therapeutic interventions across a range of clinical settings.
  • Demonstrated ability to manage a varied caseload, prioritise competing demands and work with increasing autonomy.
  • Excellent communication and relationship-building skills with consumers, carers, multidisciplinary teams and community agencies.
  • A commitment to continuous professional development, quality improvement and person-centred, trauma-informed practice.
  • A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups.
  • A current Working With Children Check (WWCC).
  • A current NDIS Worker Check.
  • A current South Australian Driver's Licence and willingness to travel throughout the Limestone Coast region.
  • Immunisation Risk for this position is Category A.
  • SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy.
