Requirements

• Master of Social Work degree from an accredited School of Social Work required.
• 2 years of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization required.
• Broad knowledge of the principles of social work and the ability to apply them with individuals, families and/or groups integrating knowledge of normal development and the aging process, changes related to illness, and the needs of patients with acute, chronic, and terminal illness in assessment and intervention with attention to cultural and literacy implications.
• Knowledge of medical, mental health and substance use diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology
• Strong organizational, communication and collaboration skills, including an ability to effectively triage patient and family needs, function as member of an interdisciplinary team, identification and collaboration with community stakeholders or resources and written documentation.
• Knowledge about social determinants of health, principles of diversity/equity/inclusion and culturally responsive practice with populations who have experience historical marginalization
• Demonstrates ability to locate, utilize and advocate for patient/family access to hospital/ community resources.
• Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues.
• Ability to navigate electronic medical records and effectively utilize basic computer programs.
• Knowledge of legal, regulatory and compliance requirements and an ability to be a resource to all team members regarding such issues and knowledge of current health care environment.
• Oregon: Licensed Clinical Social Worker (LCSW).
• Washington: Licensed Independent Clinical Social Worker (LICSW)
• If the employee is not licensed in both states at time of hire, application must be submitted to the licensing agency, in the state the employee is not yet licensed in, before the employee’s start date. Licensure in both states is required within 6 months of hire.
• CPR certification required: American Heart Association (AHA) issued BLS Provider certification.

Responsibilities

• The primary function of this job is to be available and flexible to float to various sites throughout Legacy Health System and provide Care Management Social Work services.
• Provides social work services in a variety of adult and pediatric settings across the health care continuum with a focus on holistic patient-centered care and attention to the needs of vulnerable populations. Works both independently and collaboratively as part of an interdisciplinary team.
• Key services include (but are not limited to) comprehensive evaluation of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end of life issues with the use of brief counseling, transition planning, crisis management and planning and coordination with internal and community stakeholders. Social work intervention may be with individuals, family members or groups.
• Throughout the care continuum, social workers will utilize a strengths-based approach with the aim to enhance the capacity of people to address their own needs.
• This job is paid at a flat rate plus appropriate differentials while working.
