About The Position

The Social Worker will create a valuable and meaningful experience for tenants to receive services needed for the ability to live in one’s home independently. The Social Worker will provide social service support to the staff of POP Management. The Social Worker will provide services throughout the residence through consultation, assessment and modeling. Social Worker will liaise with social service staff and Catholic Charities Brooklyn and Queens’s portfolio to further develop the individual, group, and community with interventions to meet the needs of tenants. The Social Worker will enhance the Team by performing interventions, coordinate care and services with social service teams in the buildings. The Social Worker will develop groups and assist in bringing the communities and resources together already developed in the senior properties and regions. Social Worker will work in a team approach with Clinical Director to find effective approaches and evidence based practices to assist staff in their regions. The Social Worker will ensure the Agency Mission and Strategic Goals are upheld and modeled for other staff, while also promoted to tenants and the broader community in which our programs work.

Requirements

  • Master’s in Social Work, Mental Health Counseling, Human Services, Psychology or related field
  • Demonstrated experience working with seniors developing social, educational, and health related programs.
  • Demonstrated experience in care coordination/crisis intervention with seniors.
  • Strong ability to motivate others.
  • Excellent organizational, verbal, & written communication skills.
  • Ability to facilitate on site meetings with CCBQ programs and outside agencies.
  • Ability to assess health, mental health, wellness, and housing needs.
  • Computer literate, including experience with all Microsoft Office software packages, local area networks, and the Internet.
  • Ability to work as a team member.

Responsibilities

  • Ensure the development of appropriate education and social supportive programming is consistently taking place at senior buildings in coordination with on-site housing.
  • Facilitate coordinating care on targeted critical/high need cases with social service coordinators.
  • Work through a team approach in a partnership with Regional Property Manager, Social Service staff and recertification specialist and maintenance staff to identify and respond to targeted tenants in need.
  • Conduct and facilitate field visits with Social Service Coordinators.
  • Conduct and attend case conferences with tenants, families and Social Service Coordinators.
  • Document all workshops, meetings and tenant contacts.
  • Accompany tenants with Social Service Coordinators to face to face meetings as needed.
  • Facilitate/co-facilitate case conferences, groups, workshops in senior buildings.
  • Provide coverage in the absence of Social Service Coordinator or Social Worker.
  • Conduct one-on-one training with Social Service Coordinator/Social Worker.
  • Serve as a liaison between multiple social service projects in senior buildings.
  • Conduct home visits with social service coordinator to assess safety.
  • Meet with Social Workers and Social Service Coordinators on site to follow up on critical needs cases and provide direction to social service partnerships.
  • Report and assess risk management issues to Clinical Manager.
  • Respond to crisis and emergencies in collaboration with and in absence of Clinical Manager.
  • Strictly adhere to and enforce all the Agency policies regarding Confidentiality and Corporate Compliance regulations and maintain compliance with HIPPA regulations regarding the privacy act.
  • Adhere to compliance with fire, safety, and health standards per POPM.
  • Liaise with CCNS departments, including Integrated Health and Wellness, Older Adult, Family and Community Services, Community Programming, Office of Mission, as appropriate.
  • Support to enhance social service team to consult on the activities and events.
  • Attend weekly/monthly meetings.
  • Participate in in-service trainings related to management and/or integrated health/wellness.
  • Participate in CQI initiative and provide that information to Clinical Manager.
  • Complete reports and case summaries as needed.
  • Attend and participate in supervision, workshops, classes, & in-service trainings on a regular basis to maintain current information on eligibility requirements for all basic entitlements.
  • Provide support to Social Service Staff in the absence of Clinical Manager.
  • Perform other duties as assigned.
