About The Position

An employee in this position provides direct casework and counseling services to the patient/family unit wherever they may be within the hospice program of care. The social worker will provide an assessment of psychosocial problems; define treatment goals and work to alleviate such problems. The social worker serves as part of the interdisciplinary team and shares responsibility for other aspects of patient care except where limited by licensure. Plans and coordinates patient care with the primary nurse under the direction of the Counseling Supervisor, Executive Director, and/or Regional Counseling Supervisor.

Requirements

  • Graduate of a Bachelor’s or Master’s program in Social Work accredited by the Council of Social Work Education.
  • Licensed by the state of Kentucky as a Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of social service experience in a health care setting or social service agency.
  • Knowledge of the process of death, dying, and bereavement.
  • Knowledge and basic understanding of the psychosocial and legal aspects concerning terminally ill patients and their families.
  • Knowledge and basic understanding of the interpersonal dynamics within an interdisciplinary healthcare team.
  • Current Kentucky driver's license and unlimited access to a reliable automobile.
  • Ability to exercise judgment and discretion in confidential matters.
  • Ability to make home visits to patients/families in various settings (mobile homes, apartments, homes, nursing facilities, condominiums, which may not have elevators).
  • Ability to drive for extended periods of time.
  • Flexibility.
  • Good written and oral communication skills.
  • Ability to give and receive constructive feedback.
  • Compliance with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
