About The Position

Within the Prevention Social Worker position, you will be facilitating solution-focused interventions intended to address challenges associated with a behavioral health need and to achieve identified goals or objectives as set forth in the family’s service plan. Activities provided under prevention services are intended to assist the child/youth and family/caregivers to achieve stability and foster improvement in daily living, personal recovery, resiliency, family and interpersonal relationships, school relationships and create community integration, with an effort towards achieving/maintaining permanency for all children within the family. Services are delivered in the family’s home, or other community settings, and are goal-directed based on the individual needs of that family. The service delivered by the Prevention Social Worker will be in conjunction with other services being delivered to the youth and family by the Case Planner, Youth Development Specialist, and Family Advocate, as well as other community providers. As such, Prevention Social Worker staff must work in a coordinated manner with both agency and outside providers.

Requirements

  • Master’s degree in social work, psychology, or in related human services field, plus one year of applicable experience; OR Exception may be considered for a candidate in their final year of a master’s program with the expectation of completion within one year of hire.
  • A clean and valid New York State Driver’s License; and be actively licensed for at least one (1) year - is required.
  • Local travel is required to provide youth services; therefore, it is essential to have a reliable (fully-insured) vehicle with proof of 100/300 liability coverage.
  • Ability to work flexible hours to accommodate client schedules
  • Proficiency in computer applications: Microsoft Office Suite (Word, Excel, PowerPoint), Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.), and use secure Internet practices.
  • Self-motivated and engaged, with a strong attention to detail; Excellent writing aptitude.
  • Qualities of positivity, flexibility and adaptability to day-to-day changes with client needs and functioning; And a professional record of integrity, proven reliability and dependability.
  • Capable of exercising ethical, independent judgment and discretion, and handle sensitive data with strict confidentiality.
  • Ability to complete pre-employment background check requirements, including CBC fingerprinting.
  • Staff in driving positions must be able to meet the agency’s minimum requirements of our insurance policy for authorized drivers including, but not limited to, the minimum age of 21 at time of hire; with the ability to maintain insurability throughout employment.

Responsibilities

  • Engage directly with youth and families in their home and community to provide 1-1 teaching, coaching, psychoeducation, and rehabilitative counseling to build the skills and capacities of a families struggling with mental health and developmental challenges.
  • Provide evidence-based services as identified on the family’s Treatment/Service Plan following the specific frequency, scope, and duration while being in accordance with Prevention Services regulations.
  • Regular and ongoing communication with other providers to advocate for the family’s needs, mental health wellbeing and any non-healthcare needs (ex. housing, employment, social support).
  • Facilitation of groups at agency location consisting of topics relevant to our population (parenting trauma recovery, peer support, etc)
  • Clinician will adhere to the CASSP Core Principles, affirming a client’s right to self-determination and full involvement with the treatment team in terms of developing service goals and consumer friendly methods for attaining these goals; assuring that services are child centered, family focused, culturally competent and community based.
  • Collaborates with staff across all agency programs to ensure quality environments and adherence to regulations.
  • Meets bi-weekly with Associate Executive Director of Clinical Services for clinical supervision
  • Serves on Agency committees as requested by Director of Prevention Services.
  • Timely and accurately document the delivery of services in an electronic health record.
  • Local travel required to provide the services in the family home, or other community settings.
  • Maintain a positive, strength-based and person-centered approach at all times.
  • Additional position-related duties and responsibilities as assigned by the supervisor.
