Provides social work services to patients, their families and/or significant others through counseling, emotional support, assisting with environmental needs, crisis intervention, and group leadership. This role will work closely with primary care practices and the Population Health team to provide patient centered support, connect individuals with appropriate community resources, and help address social needs that may impact their health and well being. Although this position is remote, candidates who reside in or are familiar with the Northern Panhandle of West Virginia (e.g., Wheeling, Moundsville, and surrounding communities) are preferred. Familiarity with the region and its community resources will be valuable in supporting local primary care practices and the patients served.
