Requirements

  • MSW Social Work from an accredited school of social work.
  • Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LCSW-C level.
  • Must have Epic Experience
  • Minimum of 2 years of experience in providing clinical social work service in a Oncology, pallative care, or Hospice Hospital setting is required.
  • Experience with hospital discharge planning is a requirement.

Responsibilities

  • Conduct comprehensive biopsychosocial assessments
  • Identify patient needs and develop individualized care plans and interventions
  • Provide crisis counseling, suicide assessment, and short-term counseling
  • Support care coordination and address social determinants of health
  • Assist with complex discharge planning and mitigate psychosocial barriers
  • Facilitate patient and family education and engagement
  • Lead and participate in family meetings and advance care planning discussions
  • Provide support around palliative care, end-of-life planning, grief, and loss
  • Collaborate with interdisciplinary teams to optimize patient care
