About The Position

This position is for a Social Worker - Non Certified in a healthcare setting, specifically within Long Term Care, Short Term Rehab, and Skilled Nursing facilities. The role involves providing therapeutic intervention, maintaining documentation, developing care plans, and acting as a liaison between residents, families, and external agencies. The Social Worker will also be involved in discharge planning, facility tours, and coordinating admissions.

Requirements

  • 1 year of Social Work Experience
  • Long term care/skilled nursing experience preferred

Responsibilities

  • Participate in an interdisciplinary team approach to better serve the needs of the Center population.
  • Provide therapeutic intervention to help residents cope with the social and psychological aspects of their illnesses, impairments, or disabilities, as well as their feelings about institutionalization and separation from family, community, and physical and emotional loss.
  • Maintain written documentation in the resident medical record per the company's policy.
  • Perform an initial evaluation/assessment of each resident's psychosocial needs to be placed on the medical record. Written documentation at least quarterly is required for each resident.
  • Provide ongoing written documentation for short-term residents.
  • Develop the social service component of the resident's plan of care, identifying specific problems, goals, and approaches.
  • Participate in and coordinate interdisciplinary care planning conferences at the time of admission and at required intervals to assist in developing and revising the overall plan of care for residents.
  • Ensure and document that residents and/or families have input into the care planning process.
  • Act as a liaison between residents, families, outside agencies, and the Center Administrator to ensure resident rights are maintained and upheld.
  • Conduct family meetings as needed, such as new admission group meetings, support groups, and family and friends council meetings.
  • Facilitate discharge planning, including the development of an organized discharge plan for all residents.
  • Concern oneself with the safety of all Center customers to minimize the potential for fire and accidents.
  • Coordinate admissions to the facility through generating referrals.
  • Develop and maintain relationships with area referral sources.
  • Verify insurance coverage through computer systems or verbal authorization.
  • Conduct tours of the facility for all outside interested parties.
