Provides skilled interventions for the support and/or resolution of patient and family crises, problem-solving and decision-making, advocacy and facilitation necessitated by life-changing events (adoption, guardianship, abuse, placement, bereavement), mediation of risk factors and coping with disease or disability, and other psychosocial and/or socioeconomic issues while adhering to Saint Francis Health System and departmental policies/procedures. This position is ECB status, requiring a minimum number of worked hours per month as needed by the department, with limited benefit offerings. The role involves receiving referrals, assessing patients and families, integrating social work plans into patient care, providing case management, discharge planning, continuing care services, advocacy, crisis intervention, and patient/family education. It also includes addressing abuse and neglect, advanced directives, psychiatric placements/issues, and end-of-life issues. The Social Worker will utilize hospital and community resources, facilitate insurance coverage, document care plans, complete statistical reports, and collaborate on discharge and transitional planning. Professional development is encouraged through supervision, conferences, and inservice training. Participation in community education and support group activities is also part of the role. This position requires independent judgment in decision-making and coordination of activities without direct supervision. It is a safety-sensitive position under the Oklahoma Medical Marijuana Use and Patient Protection Act.
