About The Position

Lehigh Valley Health Network (LVHN) is seeking a Social Worker with a Master of Social Work (MSW) to provide psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis. This role involves collaborating with the management team to ensure efficient resource utilization for positive patient outcomes. Requirements

  • Master’s Degree in Social Work
  • 1 year experience working as a social worker or applicable mental health experience.
  • Ability to work effectively with people from a broad range of backgrounds.
  • Ability to adhere to the National Association of Social Workers Code of Ethics.
  • Knowledge of patient rights and laws relative to those rights, such as HIPAA.
  • Knowledge of psychological principles and theories.
  • Must possess a high degree of emotional intelligence and empathy towards others.
  • Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.

Responsibilities

  • Employs counseling skills to help patients/families understand, accept, and follow recommendations for care, and to restore patients to optimum social and health adjustment.
  • Stays up-to-date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance.
  • Intervenes in crisis situations, effectively applying crisis intervention theory and techniques.
  • Identifies and records clinical, psychosocial, and financial barriers to a smooth transition across the healthcare network.
  • Participates in monthly or yearly audits or reviews to ensure adherence to laws and other federal guidelines.
  • Works collaboratively with patients and their social support systems to establish and implement a plan.
  • Interviews patients to complete the initial psychosocial evaluation.
  • Reports suspected cases of child and adult abuse/neglect/exploitation.
