Requirements
- MSW (Master of Social Work) degree.
- National Social Workers Association’s Code of Ethics adherence.
- Proficiency in utilizing EMR for documentation.
- Experience with DPA/POSLT coordination.
- Ability to perform Advance Care Planning discussions.
- Experience with Complex discharge planning.
- Ability to perform Bio-psychosocial assessments.
- Ability to perform a psychosocial assessment and reassessment on patients (pediatric, adolescent, geriatric, and general patient population).
- Ability to communicate appropriately and clearly to physicians and all ancillary services.
- Clinical judgment in making recommendations.
- Experience with crisis intervention.
- Experience with individual and family counseling.
- Experience coordinating discharge for complex cases and under-resourced patients.
- Experience assessing behavioral health patients.
- Experience arranging evaluations, 5150 hold, input psych transfers and/or arrange to output.
Responsibilities
- Development, planning, implementation, and evaluation of all social service-related interventions for corresponding Department/s.
- Identifying patients who have psycho/social needs on an inpatient or outpatient basis.
- Performing social work assessments and interventions as needed for all respective assigned patients.
- Performing comprehensive assessments and assisting with developing a plan that integrates medical and social resources as a member of the interdisciplinary team.
- Crisis intervention, patient/family intervention, high-risk screening, and grief counseling.
- Referring to identified resources and engagement of appropriate agencies or community resources when high-risk patients are identified.
- Establishing and maintaining professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients/patient representatives.
- Active participation in Department's continuing education/staff development programs.
- Involvement in program planning in assigned service areas along with the organization of social work activities.
- Providing social work services to patients and their families with psychosocial needs.
- Adhering to the National Social Workers Association’s Code of Ethics; requesting and utilizing supervisory consultation as needed and departmental standards dictate.
- Utilizing tool/templates in EMR for Social Work consults, family counseling, and group sessions for documentation.
- Performing triage for patients within corresponding team(s) and assisting in coordinating patient care delivery, including DPA/POSLT and documenting pertinent information in the case management system.
- Initiating and/or participating in team care conferences to include patients, family, and care providers.
- Following up on communication to the referral source, IE, PCP, family member, case manager, home health personnel, community social worker, Health Plan, etc.
- Performing Advance Care Planning discussions, Complex discharge planning, and Bio-psychosocial assessments.
- Referring patients to agencies that provide supportive services optimizing patients’ health plan benefits.
- Providing alternatives for patients requiring specific services.
- Performing a psychosocial assessment and reassessment on patients, including pediatric, adolescent, geriatric and the general patient population.
- Identifying and addressing psychosocial needs of patients and family.
- Communicating appropriately and clearly to physicians and all ancillary services.
- Consulting other departments as appropriate to collaborate in patient care and performance improvement activities.
- Assessing behavioral health patients and arranging evaluations, 5150 hold, input psych transfers and/or arrange to output.
- Following up as appropriate.
- Utilizing clinical judgment in making recommendations that will assist in addressing psychosocial barriers to treatment and discharge planning, including crisis intervention and individual and family counseling.
- Coordinating discharge for often complex cases and under-resourced patients.
