About The Position

Sentara Quality Care Network (SQCN) is hiring a Social Worker, MSW, to join the Population Health Team. This role contributes to value-based care by collaborating with and serving as an expert consultant to Population Health RNs, providers, and practice staff. The Social Worker will engage in management approaches to optimize health outcomes, acting as an advocate for patients, families, and significant others. They will serve as a liaison between providers, patients, care team members, skilled nursing facilities, home health, hospital staff, and community partners/resources. Additionally, the Social Worker will act as a resource to assist patients needing community-based program assistance and empower them with the skills to navigate the process for obtaining appropriate medical care and community assistance.

Requirements

• Master’s degree in Social Work REQUIRED
• 2 years' experience in direct patient care and case management REQUIRED
• Candidates must reside within a 30-minute driving distance of Harrisonburg, Rockingham, Mt. Jackson, Broadway, Staunton, and Elkton, VA.
• Travel may be required based on patient needs or assigned cases.

Nice To Haves

• Experience working with the frail or elderly population preferred
• Knowledge of Medicare, Medicaid and CMS regulations preferred

Responsibilities
