About The Position

This is a per diem/on call position that will serve participants in south Snohomish county to south King county. As a member of the interdisciplinary team, the Social Worker provides social work services to participants in a manner consistent with the mission and core values of Providence Health System. The role involves spending a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, the Social Worker provides perspective on the social, emotional, cognitive, and ethnic factors that impact the creation of an individualized integrated plan of care. The role also involves accessing community and financial resources for participants and ensuring continuity of care across the continuum. Social work assessments and interventions may also be conducted in other care settings, including the participant's home, as needed. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits, and more. The team aims to empower elders in the community to live active, independent lives.

Requirements

  • Master's Degree in Social Work
  • Washington Clinical Independent Social Worker Associate License upon hire, OR Washington Clinical Independent Social Worker Associate Temporary License upon hire, OR Washington Clinical Independent Social Worker License upon hire, OR Washington Clinical Independent Social Worker Temporary License upon hire.
  • 1 year of work experience with geriatric population
  • 2 years of Social work with geriatric age groups from diverse ethnic cultures.

Responsibilities
