About The Position

The Social Worker MSW will be responsible for care coordination, including rounding with interdisciplinary teams to facilitate patient progression of care, consultation, collaboration, and identification of patients requiring further evaluation. This role involves developing and implementing discharge plans, addressing complex psychosocial issues, and conducting discharge planning evaluations. The Social Worker will work collaboratively with the Care Transitions Nurse, patients, and their support systems to create and contribute to care plans. Additionally, the position requires providing social work interventions, educating patients and family members, maintaining effective communication with team members, and developing community relations.

Requirements

  • MSW from accredited Graduate School of Social Work.
  • NJ Social Work License: LCSW or LSW.
  • NPI and Medical Non Billing Provider Number are required; the application should be submitted to the State within the first 30 days of employment.
  • EARC-PAS certified within 3 months of hire when applicable to assignment.

Nice To Haves

  • Previous hospital experience preferred.
  • Experience in medical social work/perinatal pediatrics/behavioral health/case management preferred.

Responsibilities

  • Round with interdisciplinary team members for the purpose of progression of care, consultation, collaboration and identification of patients requiring further evaluation, development or implementation of discharge plans or complex psychosocial issues
  • Conduct discharge planning evaluation and coordination of care
  • Document clearly, concisely and relevantly
  • Work with the Care Transitions Nurse, patient, and/or formal/informal support system to develop and/or contribute to a care plan appropriate for the patient
  • Provide Social Work intervention when appropriate
  • Educate patients and family members
  • Maintain effective collaborate communication with interdisciplinary team members
  • Develop and maintain community relations
