About The Position

This position requires the full understanding and active participation in fulfilling the mission of Orange Coast Memorial Medical Center. The employee is expected to demonstrate behavior consistent with core values: Integrity, Accountability, Best Practices, Compassion and Synergy, and support the medical center’s strategic plan, participating in performance improvement/patient safety activities. The role involves seven days a week, including holidays, "on call" coverage on a rotating basis. The Social Worker provides social work services to patients and families in all hospital areas, addressing psycho-social issues related to acute or chronic illness and/or disability. Responsibilities include providing interventions to remove psycho-social barriers for safe and timely transition through the continuum of care, advocating for patients' control over their treatment and self-determination. The Social Worker also focuses on ensuring quality care while engaging in cost-effective measures such as Performance Improvement Projects and LEAN activities. Professional services encompass biopsychosocial assessment, brief treatment, crisis intervention, patient education, discharge planning, custodial placements, and resource management.

Requirements

  • Master of Social Work (MSW) Degree from an accredited School of Social Work required.
  • Current American Heart Association Basic Live Skills card

Nice To Haves

  • 1-2 years or more of direct social work experience in an acute healthcare setting preferred.

Responsibilities

  • Conducts psycho-social assessments and interventions with patients and families.
  • Addresses psycho-social issues and concerns through the continuum of care.
  • Navigates systems and advocates to assist patient in accessing appropriate resources and services.
  • Makes mandated protective services reports based on their psycho-social assessment of patient and family.
  • Addresses barriers to a safe and timely discharge.
  • Ability to be at work and be on time.
  • It is required to be on the campus of the medical center during scheduled core business hours to accommodate issues which may arise out of a crisis and to support the medical team, peers and staff and their assignment to a specific area.
  • Ability to follow company policies, procedures and directives.
  • Ability to interact in a positive and constructive manner.
  • Ability to prioritize and multitask.
